Here's a suggested formatting for entering the carnival information:

Hosting blog(s) - carnival name - carnival theme (due date for submissions; live date(s) for the carnival)

You can leave out any element that is irrelevant to your carnival.

Ex.:

Hobo Mama & Code Name: Mama - Carnival of Natural Parenting - Parenting Philosophy (due July 5; live July 12)

Be sure to enter a link relevant to the carnival — the current submissions page (http://www.hobomama.com/2011/06/calling-for-submissions-for-july.html) or a static page for the carnival (http://www.hobomama.com/p/carnival-of-natural-parenting.html), not your home page. People interested in your carnival want to be able to find the necessary information quickly.

I will manually approve each addition to make sure carnivals are relevant and that general submission rules (see above) are followed. There will be a delay between entering your information and seeing it in the carnival linky. I don't have time to email everyone about my reasons for approving an entry or not, but if you want me to be able to contact you about your submission, be sure to enter a valid email address in the linky (optional).

I will remove carnivals as their live dates have passed, so be sure to add the new dates and theme for each upcoming carnival session.
